In a nutshell: The authors of this study designed a pharmacist-led care management programme to improve blood pressure control among patients with type 2 diabetes who had persistent hypertension. The pharmacists had access to electronic pharmacy prescribing and clinical data systems and promoted adherence by use of motivational interviewing.
